Does NHS Scotland use different pay scales?
NHS Scotland uses the same Agenda for Change (AfC) framework as NHS England, with similar pay bands and spine points. However, there have been separate pay deals negotiated for Scotland, which have resulted in slightly different pay scales at various times. For 2026/27, the pay scales shown above reflect the approximate Scottish AfC rates.
The key difference isn't the salary — it's the tax.NHS Scotland workers pay Scottish Income Tax rates, which are higher than England's for most pay bands. At Band 6 maximum (~£44,962), an NHS Scotland worker pays approximately £2,100 more in income tax per year than an NHS England colleague on the same gross salary.
Does the NHS pension work differently in Scotland?
The NHS pension (1995/2008/2015 scheme for Scottish staff) has the same structure as the rest of the UK. Contribution rates and employer match are the same. What differs is the tax treatment of the pension deduction: because Scotland has different marginal rates, the tax saving from pension contributions differs for Scottish NHS staff at higher income levels.
